How to fill out form WH-347, column by column

The official DOL instructions run several pages of regulatory language. Here is what each field actually wants, in the order you'll fill it.

Header
Your company name and address, check contractor or subcontractor, payroll number (sequential from week 1 on this contract), week ending date, project name and location, and the project or contract number.
Column 1 — Name and ID
Each worker's full name and the last four digits of their Social Security number. Never the full SSN.
Column 2 — Withholding exemptions
Optional; most agencies ignore it. Leave blank unless your agency asks.
Column 3 — Work classification
The classification from the contract's wage determination that matches the work actually performed — not the job title on your org chart. A worker doing two trades in one week gets split lines. If the trade isn't on the wage determination, request a conformance (SF-1444) before self-inventing one.
Column 4 — Hours worked
Daily straight-time and overtime hours in the O/S grid. Overtime is over 40 hours in the workweek, paid at 1.5× the basic rate under CWHSSA.
Column 5 — Total hours
Sum of the daily entries per row.
Column 6 — Rate of pay incl. fringe
The actual hourly rate paid. If you pay fringe in cash, show base rate plus cash fringe (e.g. "$41.20/$9.55"). Must be at least the wage determination's base + fringe for the classification.
Column 7 — Gross amount earned
Two numbers: gross on this project, and gross for the full week across all projects (shown as a fraction, e.g. 1,648.00/2,210.00).
Columns 8 — Deductions
FICA, federal and state withholding, plus any other deductions — 'Other' must be itemized. Deductions apply to the all-projects gross.
Column 9 — Net wages paid
What actually hit the worker's check for the week.
Page 2 — Statement of Compliance
Signed by the owner, an officer, or an authorized payroll supervisor. Check 4(a) if fringe is paid into bona fide plans, 4(b) if paid in cash. The signature certifies accuracy under penalty of federal law (falsification can be prosecuted under 18 U.S.C. § 1001).
